Wendy Dean and Simon Talbot's moral injury framework, developed specifically for physicians, documents that the primary driver of physician burnout is not compassion fatigue but the moral injury of being prevented from practicing according to trained values — a fundamentally identity-level wound. Tait Shanafelt's extensive burnout research at Mayo Clinic documents that physician burnout is at epidemic levels and is disproportionately concentrated in identity-eroding practice environments. Cruess, Cruess, and Steinert's Professional Identity Formation framework was developed explicitly in the context of medical education, making it the most directly applicable theoretical framework for this extension. Maslach and Leiter's burnout dimensions all present in physicians at elevated rates. Danielle Ofri's research on physician emotion documents the gap between the clinical mask and the human reality that defines physician identity strain. Abraham Verghese's work on narrative medicine provides a counterpoint framework: physicians who maintain a strong narrative identity show greater resilience and patient connection.
